Release and Waiver
of Liability

In Consideration of being permitted to adopt an animal from Changing Tomorrow Animal Rescue, Inc. (Hereinafter “Changing Tomorrow Animal Rescue”) in Boca Raton, Florida, EACH  OF THE UNDERSIGNED, for himself/herself, his personal representative, heirs, next of kin,  invitees, and/or guests:  

1. Acknowledges, agrees, and represents that he/she understands the guidance regarding  care for recently adopted animals that has been provided, and in particular, understands  the importance of ensuring that their adopted animal must be kept away from other  animals in the household for fourteen (14) days and that all introductions to other animals  should be done with the undersigned present.  
2. Acknowledges, agrees, and represents that he/she is aware of and assumes the risks  associated with a failure to quarantine animals, including but not limited to any animals  already in the household being infected with a disease as a result of the failure not to  abide by adoption guidelines (which may then spread within the household), and that  undersigned hereby accepts responsibility for the animal(s) in their care. 
3. HEREBY RELEASES, WAIVES, DISCHARGES, and COVENANTS NOT TO SUE Changing Tomorrow Animal Rescue, its employees, agents, members, directors, officers,  or any affiliated entities thereof, FROM ALL LIABILITY TO THE UNDERSIGNED,  his personal representative, assigns, heirs, next of kin, guests, and/or invitees FOR ANY  AND ALL LOSS OR DAMAGE, AND ANY CLAIM OR DEMANDS THEREFORE  ON ACCOUNT OF ILLNESS TO THE UNDERSIGNED OR THEIR ANIMALS ARISING OUT OF OR RELATED TO THE ADOPTION OF AN ANIMAL FROM  CHANGING TOMORROW ANIMAL RESCUE AND A FAILURE TO  QUARANTINE THE ADOPTED ANIMAL. 
4. HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS  Changing Tomorrow Animal Rescue, its employees, agents, members, directors, officers,  or any affiliated entities thereof, from ANY LOSS, LIABILITY, DAMAGE, OR COST  DAMAGE ARISING OUT OF OR RELATED TO THE ADOPTION OF AN ANIMAL  FROM CHANGING TOMORROW ANIMAL RESCUE AND RESULTING FROM A  FAILURE TO QUARANTINE THE ADOPTED ANIMAL. 
5. HEREBY agrees that this Release and Waiver of Liability extends to all acts of  negligence by Changing Tomorrow Animal Rescue, its employees, agents, members,  directors, officers, or any affiliated entities thereof, and is intended to be as broad and  inclusive as is permitted by the laws of Florida and that if any portion thereof is held  invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force  and effect. 

I HAVE READ THIS ADOPTION AGREEMENT, RELEASE AND WAIVER OF  LIABILITY, FULLY UNDERSTAND ITS TERMS, AND HAVE SIGNED IT FREELY AND  VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE  BEING MADE TO ME AND INTEND MY SIGNTAURE TO BE A COMPLETE AND  UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT  ALLOWED BY LAW.
